A small clinical study from Scripps Health in San Diego has examined whether patients who reached their goal weight on GLP-1 receptor agonists can maintain that weight by reducing their injection frequency. Published in the Journal of Obesity, the findings offer a new data point in the ongoing investigation of GLP-1 dosing protocols.
The GLP-1 Dosing Dilemma
GLP-1 receptor agonists have been used in type 2 diabetes management for over two decades. Their more recent application for weight loss has raised a practical question for patients who reach their target. Stopping the medication is associated with weight regain and the return of appetite-related symptoms described as "food noise." Continuing indefinitely carries significant cost and supply considerations.
Dr. Mitch Biermann, an internal medicine physician at Scripps Clinic Carmel Valley, conducted the study after observing that some patients had experimented with their dosing intervals on their own. "When people reach the point where they have normalised their weight and reversed their metabolic problems, they often ask, 'What do I do now?'" Biermann told Scripps Health. "Drug companies have done discontinuation trials where they stop the medicine and see what happens. In those, most people regained most of their weight and reversed the metabolic improvements."
Study Design and Results
Biermann's trial tested whether patients who had achieved their goal weight could maintain it by taking their regularly prescribed GLP-1 injection every other week rather than weekly.
Of the 34 patients enrolled, 30 were able to maintain their weight loss without additional diet or lifestyle changes. Some participants lost additional weight and maintained lean mass. The findings were published in the Journal of Obesity.
The study is small and the results are preliminary. Larger controlled trials would be needed to establish optimal dosing protocols for weight maintenance.
Oral GLP-1 Medications on the Horizon
Biermann noted that the emergence of oral GLP-1 medications could expand the available options. Data suggests oral formulations may produce more modest weight loss than injectables, but the once-daily pill format could offer patients additional flexibility in managing their dosing.
The Role of Exercise
Exercise emerged as a key theme in the study's commentary. Biermann described physical activity as the most important tool for preventing weight regain. "It is paradoxical, because exercise does not cause people to lose a lot of weight upfront, but it is our number one tool to prevent regaining it," he said. Exercise raises endogenous GLP-1 levels, which can support appetite regulation.

This study contributes to a growing body of research on GLP-1 biology and dosing dynamics. Understanding whether reduced-frequency dosing can sustain physiological effects has implications for metabolic research and peptide science more broadly. The results add to the evidence base on dose-response relationships and the duration of peptide activity at reduced administration intervals.
